New insights provided regarding post-acute care discharge destinations, hospice length of stay and home health adherence rates compared to readmission rates

Key Observations Outlined in the Report
Excel Health’s public reports reveal national and state-specific trends every quarter and in this case, annual comparisons between 2016 and 2017. These reports pull from a subset of the company’s proprietary solution that includes detailed market share, performance analytics and quality metrics based on 100% of the most recent Medicare Part A and B claims data. Observations include:
- The hospice market experienced significant growth (6.5%) in admissions in 2017 when compared to 2016; with more than 1.3 million Medicare enrollees receiving hospice services. Hospice utilization rates grew over 1.5 percentage points to an average of 47.7% in 2017, compared to 46.3% in 2016. Hospice average length of stay (ALOS) averaged 62 days in 2017, but varied widely by state, which can be seen in the full report.
- The home health admissions rate remained relatively flat this year (0.5%) with more than 3.7 million traditional Medicare enrollees benefiting from home health services. There were significant discrepancies in admission rates by state, with four of the top 10 declining states having an active moratorium in place, prohibiting the opening of new agencies. On the flip side, 8 of the 10 growth states had fewer than 100 active home health agencies. These numbers indicate potential expansion opportunities in markets that are not yet saturated.
- New to the report this quarter is an analysis of discharge patterns. Nearly half (49%) of all patients who were discharged from an inpatient hospital did not receive post-acute care instructions in 2017. Of those who did, 22% were referred to skilled nursing facilities, 20% to home health, 5% to inpatient rehab facilities, 4% to hospice and 1% to long term care. Much like home health admissions rate, discharge patterns varied dramatically by state with home health varying from 8% to 30% of hospital inpatient discharges.
- In addition, the report takes a closer look at home health adherence rates. For patients who received instructions to receive home health benefits, an average of 30% did not follow through and receive the benefits. Excel Health found that patients that do not adhere to their home health instructions are 50% more likely to be readmitted than those that benefit from post-discharge skilled home healthcare. While this differential varies widely by state, every state has an opportunity to increase home health adherence and thereby reduce hospital readmission rates.
